This site allows home health agencies to set targets for the following publicly
reported quality measures:

- Discharge to Community
- Improvement in Ambulation
- Improvement in Bathing
- Improvement in Dyspnea
- Improvement in Management of Oral Medications
- Improvement in Pain Interfering with Movement
- Improvement in Status of Surgical Wounds

- Improvement in Urinary Incontinence
Home health agencies registered to use this website can view their publicly
reported quality measure scores, select appropriate targets, and track their
progress over time. Using the home health STAR site, your home health agency can
become a 'star' performer.
Registration on this site is free and available to all Medicare and/or
Medicaid certified home health agencies.
To register for the home health STAR site, please click Create an Account. Please have your Medicare/Medicaid provider number available when creating an account. If you are already registered, please click Login to sign into your account and begin using the STAR site.

This site was developed by Quality
Improvement Organizations (QIOs) under contract to the Centers for
Medicare & Medicaid Services (CMS). As QIOs, we believe that achieving high
quality is an on-going process. Our goal is to provide home health agencies
nationwide with the tools and information you need to improve the quality of
your services. We encourage you to raise the bar for quality by continually
reevaluating your quality of care and identifying areas for improvement.
Note: This site is for your home health
agency's internal use; as a result, your registration on this site is completely
confidential. Your agency's name and targets will not be shared with state
survey and certification agencies. CMS will only view aggregate or de-identified
data.
